Will the same driver come each time?

That’s a reason to self-manage. You arrange it directly with a local provider, so a standing booking with a familiar driver is realistic.

Can someone come into the appointment with me?

Yes, if you book accompanied transport rather than a drop-off. Say so, because it’s priced differently.
